Licensed Practical Nurse (LPN)
We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) for a 15-week contract. The LPN will provide direct patient care, administer medications and treatments, monitor patient conditions, document care accurately, assist with admissions and discharges, perform wound care, collaborate with the interdisciplinary healthcare team, and ensure a safe, comfortable environment for residents while following facility policies and infection control standards.
Responsibilities:
Provide direct nursing care to residents/patients.
Administer medications, treatments, and injections as ordered.
Monitor, assess, and report changes in patient condition.
Maintain accurate and timely documentation.
Assist with admissions, transfers, and discharges.
Perform wound care and other nursing procedures within the LPN scope of practice.
Educate patients and families on care plans and health maintenance.
Collaborate with RNs, physicians, and other healthcare professionals.
Follow infection control, safety, and regulatory guidelines.
Respond promptly to emergencies and support quality patient outcomes.
Active LPN license in the state of practice.
Current BLS certification.
Long-term care, skilled nursing, rehabilitation, or acute care experience preferred.
Strong communication, organizational, and patient care skills.
Ability to work independently and as part of a multidisciplinary healthcare team.
