About Sycamore Health:
Sycamore Health is a private-duty care organization established to manage direct-hire home care for two family members aging in place in Ann Arbor. The household includes a medically complex cancer patient receiving active oncology treatment and a second patient who requires dedicated daily personal care support. Sycamore Health was structured for direct employment of caregivers rather than reliance on staffing agencies, in order to build a consistent, high-quality, accountable care team.

The Role

The Private-Duty Registered Nurse provides hands-on skilled nursing care for a patient in a private household setting who is undergoing active oncology treatment. This is a substantive, full-scope clinical role, not a light companion position. The RN will be responsible for personal care and activities of daily living, health monitoring, medication administration, and active clinical engagement throughout the day.

You will work as part of a structured care team that includes a Home Care Operations Manager and a supervising clinical team. Clear communication, sound clinical judgment, and a genuine commitment to patient dignity and wellbeing are essential in this role.

Primary Responsibilities

Personal Care and ADLs:

Assist the patient with activities of daily living including bathing, dressing, grooming, toileting, and mobility
Exercise clinical judgment to proactively identify and act on steps important for patient care and health
Support safe transfers and ambulation, using proper body mechanics and any assistive devices in use
Prepare and assist with meals and snacks in accordance with dietary guidance from the clinical team
Perform light housekeeping tasks directly related to patient health and safety, including laundry, dishwashing, and maintaining clean patient living areas

Skilled Nursing and Clinical Care:

Monitor and document patient vital signs (blood pressure, pulse, temperature, oxygen saturation, weight) per the schedule established by the clinical team
Assess and report changes in patient condition, behavior, appetite, or mobility to the Home Care Operations Manager and supervising clinical team
Administer medications per physician orders and maintain accurate MAR documentation
Support the patient in following prescribed therapy routines, exercises, or medical equipment protocols (e.g., Optune device management, physical therapy exercises)
Perform wound care, skin checks, and other clinical tasks within RN scope of practice
Recognize and respond to changes in condition that may require escalation, including symptoms related to active oncology treatment

Documentation and Communication:

Complete accurate, timely care logs and medication administration records each shift
Communicate any changes in patient status or care concerns to the Home Care Operations Manager promptly and clearly
Participate in care team check-ins and collaborate with the supervising clinical team on care planning

Accompaniment and Support:

Accompany the patient to medical appointments, therapy sessions, and outings as scheduled
Provide companionship and a calm, supportive clinical presence as a consistent part of the patient's daily life

Qualifications

Required:

Active, unencumbered RN license in good standing in Michigan
Minimum 10 years of hands-on nursing experience in home health, private duty, oncology, or a related direct care setting
Experience monitoring, assessing, and documenting vital signs and patient condition changes
Strong attention to detail in documentation and clinical task execution
Reliable, punctual, and professional; able to maintain a consistent schedule
Valid driver's license and reliable transportation
Clean background check and verifiable references from prior nursing employers
Ability to travel up to 50% of the time (potential traveling with the family on pre-planned trips to Northern Michigan and potentially out of state)

Preferred:

Experience with medication administration and care planning in a private duty or home health setting
Current CPR and first aid certification
Experience supporting patients with active oncology treatment, including familiarity with monitoring for medication side effects
Experience with durable medical equipment and/or medical device support in the home setting
Experience in a direct-hire or private household employer model
