Make a Difference as a Hospice RN in Valencia!

We are seeking a compassionate and skilled Hospice RN who is called to provide meaningful, patient-centered care in the homes of those we serve. This is more that a job - it's an opportunity to truly make a difference in the lives of patients and their families during one of the most important times in their journey.

What You'll Do

- Provide in-home hospice nursing care with a focus on comfort, dignity, and quality of life.

- Perform comprehensive patient assessments and develop individualized care plans.

- Manage pain and symptoms with clinical expertise and compassion.

- Support and educate patients and families throughout the end of life process.

- Collaborate closely with an interdisciplinary team including physicians, social workers, aides, and chaplains.

- Maintain accurate and timely documentation.

- Active CA RN license

- Hospice or Home Health experience preferred (but not required for the right candidate)

- Strong clinical judgement in pain and symptom management

- Excellent communication and emotional intelligence

- A genuine passion for end-of-life care and supporting families

- Employment is contingent upon successful completion of a background check and any required screenings in accordance with state regulations.
