Physical Therapist

Job Title: Physical Therapist (PT) – Home Health
Location: Ashland, KY 41101
Hours: 40 hours/week | 8 hours/day
Schedule: Monday–Friday 8:00 AM – 5:00 PM
Mileage Reimbursement: $0.49 per mile
Perform physical therapy evaluations and treatments in accordance with physician orders and referrals
Complete comprehensive patient assessments using appropriate evaluation tools and procedures
Review patient diagnoses, medical history, precautions, and contraindications
Develop individualized, measurable treatment plans to maximize patient independence and functional outcomes
Reassess patient progress regularly and adjust goals and treatment plans as clinically indicated

Minimum 1 year of home health physical therapy experience
Prior OASIS documentation experience
Experience with Homecare Homebase (HCHB)
License/Certification:
