As a registered nurse case manager, you would be responsible for delivering high quality nursing care to a variety of clients, completing comprehensive assessments, supervising home health aides, and completing documentation for clients that follow federal, state and local guidelines. Home health care does require in field working times and in office documentation time. This position requires you to be self motivating with precise assessment skills. You will communicate with the clients/caregivers, doctors, other staff members, and any other entities in the home to ensure appropriate and coordinated care for our clients.
