REQUIREMENTS:

* Active LPN license in Tewksbury.
* 1+ year experience in LTC/SNF preferred
* Current BLS certification
* Familiarity with EMRs (e.g., PointClickCare) preferred
* Strong communication and teamwork skills

RESPONSIBILITIES:

* Deliver routine nursing care and support daily health needs
* Administer medications and treatments as prescribed
* Monitor and document changes in resident condition
* Assist with ADLs and mobility as needed
* Maintain accurate records in EMR systems
* Communicate with residents, families, and interdisciplinary teams
* Support infection control and safety protocols
* Report concerns or changes to RN/supervisor promptly
* Participate in shift handoffs and team meetings
