Equity Staffing Group is seeking an experienced Travel Licensed Practical Nurse (LPN) to join a leading home health organization serving patients throughout the Fort Worth service area.

This position is ideal for a compassionate and skilled nurse who enjoys working independently while providing high-quality care to patients in their homes. The Travel LPN will perform a variety of skilled nursing visits and treatments while collaborating closely with physicians, registered nurses, patients, and caregivers.


The Travel LPN provides direct patient care in accordance with physician orders, established care plans, and agency policies. This role requires daily travel throughout the Fort Worth service area to deliver skilled nursing services and support patients in achieving optimal health outcomes within their home environment.

Essential Responsibilities
Perform skilled nursing visits in patient homes
Provide patient and caregiver education regarding disease management, medications, treatments, and home care plans
Conduct comprehensive patient assessments and report changes in condition to the clinical team
Perform wound care and dressing changes according to physician orders
Manage and maintain wound vacuum (Wound Vac) therapy
Perform venipuncture procedures and specimen collection
Provide care and maintenance for PICC lines and other vascular access devices
Manage PleurX drainage systems and related patient education
Administer treatments and medications as ordered by the physician
Accurately document patient visits, assessments, and interventions in the electronic medical record
Collaborate with physicians, registered nurses, therapists, and interdisciplinary care team members
Ensure compliance with all home health regulations, infection control standards, and agency policies
Travel daily between patient homes within the assigned coverage area

Active Licensed Practical Nurse (LPN) license
Minimum of one (1) year of recent Home Health field clinician experience
Current CPR certification
Valid driver's license and reliable transportation
Ability to travel throughout the Fort Worth service area
Strong clinical assessment and patient education skills
Excellent communication and documentation abilities

Preferred Qualifications
Experience with Homecare Homebase (HCHB)
Previous experience with wound care, wound vac management, PICC lines, and PleurX drains
Home health documentation experience
Knowledge of Medicare home health regulations
