REQUIREMENTS:

* Active LPN license in Hart.

* Minimum 1 year of LTC or similar experience preferred

* Current BLS certification

RESPONSIBILITIES:

* Provide direct nursing care under the supervision of an RN or physician

* Administer medications and basic treatments according to care plans

* Assist with daily living activities and resident hygiene

* Observe and report changes in residents’ conditions

* Accurately document nursing interventions and outcomes

* Collaborate with the care team to support resident-centered care

* Adhere to infection control and safety protocols
