CentralCare Incorporated is seeking a highly skilled and experienced Registered Nurse - Disease Management to provide full-time services at Joint Base San Antonio (JBSA-Lackland). This is a great opportunity to support our military community and their families as federal civilian contractors. 

COMPENSATION & BENEFITS:

Pay: $40.00/hour + $5.42/hour additional for health and wellness

Benefit: Medical, Dental, Vision, PTO, Sick Leave

Availability: Full Time

Contract Length: 1 year Preferred (opportunity to extend)



Responsibilities:

Coordinate and manage care for patients with chronic diseases and complex health conditions.

Conduct patient assessments, health screenings, and telephonic evaluations.

Develop, implement, and monitor disease management and patient care plans.

Educate patients, families, and healthcare staff on disease prevention, treatment, and wellness programs.

Collaborate with physicians, case managers, and interdisciplinary teams to improve patient outcomes.

Track patient progress, analyze population health data, and support quality improvement initiatives.

Maintain accurate documentation, reports, and compliance with military treatment facility policies and clinical guidelines.

Qualifications:

Education: Bachelor of Science in Nursing

Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE)

BLS Certified

Minimum 4 years of nursing experience managing patients with chronic and complex medical conditions after earning a BSN.

Experience conducting patient assessments, disease management, care coordination, patient education, and population health initiatives across diverse patient populations
