RN to support clinical and population health services in the Sonora, TX community.
This position focuses on improving health outcomes through prevention, assessment, health education, and collaboration with individuals, families, healthcare providers, and community partners.
Assess the health needs of populations, including health disparities, inequities, and unique needs of specific sub-populations.
Conduct comprehensive and systematic population health assessments.
Identify and address social, environmental, and other determinants of health affecting individuals and communities.
Promote primary prevention, wellness, and health improvement strategies.
Develop and implement interventions at multiple levels, including individuals, families, communities, and health systems.
Provide health education and community outreach based on priorities identified through ongoing population health assessments.
Collaborate with healthcare providers, community organizations, and other stakeholders to support improved health outcomes.
Maintain accurate documentation and comply with applicable clinical and organizational standards.
Requirements
Active Texas Registered Nurse (RN) license required.
Public health, community health, or clinical nursing experience preferred.
Strong communication, assessment, organizational, and collaboration skills.
Ability to work effectively in rural and/or underserved communities.
Ability to work independently while collaborating with multidisciplinary teams.
