On-Call Home Health & Hospice Nurse (Triage Experience Required)

$50,000 Salary + Mileage Reimbursement | 7 Days Every 14-Day Pay Period | Nights & Weekends

We are seeking a highly experienced Home Health & Hospice Nurse to join our team as an On-Call Nurse. This is an excellent opportunity for a seasoned clinician who is confident handling after-hours triage, urgent patient needs, and late-night calls independently.

Position Highlights
-$50,000 annual salary
-Mileage reimbursement
-Work 7 days per 14-day pay period
-Approximately 4–8 patient visits per pay period
-Primarily overnight and weekend on-call coverage
-Responsible for the entire service area covered by the company
-Must be comfortable responding to late-night and after-hours calls

On-Call Schedule

Week 1:
Monday & Tuesday – On Call
Wednesday & Thursday – Off
Friday, Saturday & Sunday – On Call

Week 2:
Monday & Tuesday – Off
Wednesday & Thursday – On Call
Friday, Saturday & Sunday – Off

The schedule then rotates back to Week 1.

During the week, the nurse will be responsible for taking after-hours phone calls from closing until the office opens the following morning.

Responsibilities
-Provide after-hours clinical support to Home Health & Hospice patients and families
-Perform thorough telephone triage and determine appropriate interventions
-Make in-person visits when clinically necessary
-Respond to urgent patient and caregiver concerns
-Coordinate with physicians, clinical staff, and emergency services when appropriate
-Assess changes in patient condition and determine the appropriate level of care
-Provide education and reassurance to patients and families
-Complete timely and accurate clinical documentation
-Travel throughout the company's entire service area as needed

Qualifications
-Significant Home Health and/or Hospice experience required
-Strong triage experience is essential
-Excellent clinical judgment and assessment skills
-Comfortable working independently with limited supervision
-Must be comfortable receiving late-night and weekend calls
-Reliable, responsive, and able to manage urgent situations calmly
-Valid nursing license and required credentials
