Coverage: Ruby/Garnet (Brentwood, Beverly Hills, Hollywood, West Hollywood and Los Feliz). May be asked to help in other territories within facility's service area as needed which is about a 60 mile radius from the Facility address.
Average 4-6 visits per day
Home Health and Oasis experience required
Homecare Homebase documentation experience required Client may consider without HCHB exp at a reduced rate for the first 2 weeks if the manager is willing to train.

Education: Bachelor’s, Master’s, or Doctorate in Physical Therapy (DPT) from an accredited program.
Licensure: Active Physical Therapist license in [State].
