Requirements

RN Case Manager:
Hospice experience required, at least 1-2 years
Homecare experience will suffice if there is Epic experience
Panel interview via teams or zoom
Candidate needs to be prepped for interview, any red-flags are an immediate no-offer.

• 2+ Years’ Experience; as well as experience being a supervisor
• DHHS Background
• BCLS
• National/State/County Background
• 10 Panel Drug Screen
• 2 Step TB, Hep B, MMRs, Tdap, Varicella
