Essential Duties and Responsibilities
Provide and coordinate nursing care to assigned residents in accordance with facility policies and standards of nursing practice.
Assess residents’ physical and mental health status; develop, implement, and evaluate individualized care plans.
Administer medications and treatments as prescribed by the physician and in accordance with nursing standards.
Monitor and document resident condition changes; communicate pertinent information to physicians, families, and interdisciplinary team members.
Supervise and direct CNAs and other support staff to ensure quality resident care.
Participate in admission, transfer, and discharge processes.
Ensure accurate and timely documentation in electronic health records (EHR).
Maintain infection control and safety standards within the unit.
Participate in care conferences, quality improvement initiatives, and staff education.
Respond promptly and effectively to emergency situations.
Serve as a resource and advocate for residents and their families.
Current, unencumbered Registered Nurse (RN) license in the state of employment.
Graduate of an accredited school of nursing (Associate’s or Bachelor’s degree preferred).
Minimum of 1 year of nursing experience, preferably in a skilled nursing or long-term care setting.
Current CPR/BLS certification.
Strong clinical assessment, organizational, and leadership skills.
Compassionate, patient-centered approach to care.
Proficiency in electronic medical record (EMR) systems preferred.
