Care Manager in Hunt Valley, MD

The Option Group is seeking a Full-Time Nurse Care Manager. The ideal candidate will possess a nursing degree and have case management and/or case coordination experience. We offer medical, dental, vision benefits, 401(k), paid holiday and PTO.
1. Conduct initial clinical assessments to discern the client situation and inform the family of recommendations and next steps.
2. Research and identify appropriate care settings and support services needed for clients to ensure proper services are implemented.
3. Write reports that are clear, accurate, concise and relevant in an efficient manner focusing on outlining the value of a Care Manger.
4. Advocate on the client's behalf to receive services, entitlements, benefits or the best care scenario for the client, working collaboratively with State and Local agencies and other external partners.
5. Coordinate client care functioning as a liaison between client, family, physician other service providers, etc., facilitating meetings and having difficult conversations.
6. Perform follow up visits and engagement visits on an ongoing basis to monitor client status.

BSN, RN, or LPN
5 years' experience in related clinical setting
1–2-year Care Management Experience
Valid, current state Driver's License and a minimum car insurance requirements.
