DUTIES & RESPONSIBILITIES:

* Providing compassionate care, strength and courage to patients and families.
* Helping patients identify their own personal goals of care and helping caregivers have confidence in caring for their family member.
* Acting as generalists on all diseases and specialists in symptom management, requiring excellence in assessment and critical thinking.
* Managing an assigned caseload of patients and leading the interdisciplinary team caring for those patients.
* Providing care wherever patients call home, including private homes, nursing homes, group homes, or assisted living facilities.
