About the Opportunity
A Clinical Pharmacist opportunity is available at a 267-bed acute care hospital in Fort Walton Beach, Florida. The facility is recognized for its Advanced Primary Stroke Center, Cancer Care Center, open-heart surgery services, and comprehensive emergency care through a main Emergency Department, Pediatric Emergency Department, and two freestanding Emergency Departments.

Position Summary
The Clinical Pharmacist is responsible for interpreting, verifying, and accurately processing medication orders while ensuring the safe and effective use of medications. This role collaborates with physicians, nurses, and other healthcare professionals to optimize medication therapy and support quality patient care.

Key Responsibilities
Interpret, verify, and accurately transcribe medication orders into the pharmacy computer system.

Compound and dispense pharmaceuticals accurately and safely.

Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriate dosing.

Support and maintain the hospital formulary by minimizing non-formulary procurements, utilizing therapeutic interchanges, and promoting rational drug therapy selection.

Provide clinical pharmacy services, including:

Drug consultations

Pharmacokinetic monitoring

Anticoagulation monitoring

Pain management support

IV-to-PO conversions

Renal dosing

Provide accurate medication information to physicians, nurses, and patients.

Document clinical interventions, medication errors, and adverse drug reactions in accordance with hospital policies.

Requirements

Required Qualifications
Bachelor of Science in Pharmacy (BS Pharmacy) or Doctor of Pharmacy (PharmD) from an ACPE-accredited School of Pharmacy.

Active Florida Pharmacist License in good standing.

Preferred Qualifications
One (1) or more years of hospital pharmacist experience preferred.

Candidates with PGY1 Pharmacy Residency completion will also be considered.

Day Shift

Rotating weekends

Compensation
The hiring facility offers a competitive compensation package. Final compensation will be determined by the facility based on experience, qualifications, internal equity, and organizational guidelines.
