Centerlight Adult Activity Center
The Registered Nurse (RN) plays a vital role in ensuring the health, safety, and well-being of seniors and individuals with developmental disabilities receiving services through our programs. The RN ensures that each individual receives appropriate health monitoring and support consistent with their care plan and regulatory standards. This position involves coordinating with physicians, caregivers, and interdisciplinary teams to promote wellness, safety, and quality of life for each individual served.
Job Responsibilities:
Health Monitoring & Clinical Care
- Conduct initial and ongoing health assessments for all individuals enrolled in the adult day care.
- Develop, implement, and update individual nursing care plans in coordination with physicians and program staff.
- Monitor and record vital signs, health status, and changes in condition; report significant findings to the Administrator and healthcare providers.
- Administer medications and treatments as prescribed, following proper documentation and safety protocols.
- Ensure medication administration records (MARs) and medical documentation are accurate and up to date.
Care Coordination & Communication
- Coordinate care with physicians, pharmacies, hospitals, and specialists as needed.
- Communicate medical updates and instructions to staff, individuals, and families in an understandable manner.
- Participate in interdisciplinary team meetings and contribute to individualized service plans and person-centered care planning.
- Serve as a liaison between healthcare providers, families, and program management to ensure continuity of care.
Staff Training & Oversight
- Train and supervise Direct Support Professionals (DSPs), and other staff on health-related procedures, medication administration, infection control, and emergency response.
- Provide ongoing education and refresher training to staff on health and safety practices.
- Monitor staff compliance with medication administration, documentation, and infection control procedures.
Health & Safety Compliance
- Maintain compliance with all state and federal healthcare regulations and agency policies (including HIPAA and OSHA standards).
- Ensure proper infection prevention and control measures are implemented across all care settings.
- Conduct routine audits of medical records, medication logs, and first aid supplies.
Documentation & Administrative Duties
- Maintain accurate and timely nursing notes, health assessments, and incident reports.
- Assist in preparing for inspections, audits, and quality assurance reviews.
- Support program management in developing and updating health-related policies and procedures.
