REQUIREMENTS:

* FL License (or willing to obtain) - [REQUIRED]
* Most recent NPDB Self Query Report (within 30 days active) - [REQUIRED] * no exemption, required at presentation
* Current 2+ years of employment experience as NNP - not including RN experience (note in "Experience" field) - [REQUIRED]
* Clean Malpractice History - [HIGHLY PREFERRED]
* DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) - [REQUIRED] * no exemptions
* Current 2+ years of employment experience as NNP - not including RN experience, Master's Degree (minimum) and/or Doctorate in Nursing
* Board Certified
