REQUIREMENTS:

* Basic Life Support (BLS) certification
DUTIES & RESPONSIBILITIES:
* Help patients identify their personal goals of care
* Educate and build confidence in caregivers caring for family members
* Serve as a generalist on all diseases and specialist in symptom management
* Excel in clinical assessment and critical thinking
* Manage an assigned caseload of patients and lead the interdisciplinary team
* Provide care wherever the patient calls home, including private residences, nursing homes, group homes, and assisted living facilities
