Description

We are seeking a dedicated and compassionate Wound Care Nurse Practitioner to join our healthcare team, serve the Phoenix Arizona Area . The ideal candidate will possess specialized knowledge in wound management and will be responsible for assessing, planning, implementing, and evaluating care for patients with complex wounds. This role requires a strong commitment to patient-centered care and the ability to work collaboratively with other healthcare professionals.
Duties
• Conduct thorough assessments of patients' wounds, including size, depth, and condition.
• Develop individualized wound care plans based on assessment findings and best practices.
• Administer appropriate wound care treatments, including dressings and topical medications.
• Educate patients and their families about wound care techniques and prevention strategies.
• Monitor patients' progress and adjust care plans as necessary to promote healing.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Document all assessments, interventions, and outcomes in electronic health records (Tebra).
• Provide support in critical situations, including critical care transport when needed.
• Previous experience performing skin applications and grafts.
Qualifications
• Current RNP licensed in AZ Credentialed with Medicare
• Experience in wound care management is preferred; certification in wound care is a plus.
• Familiarity and experience performing dermal fillers and skin grafting care is advantageous.
• Strong communication skills to effectively educate patients and collaborate with healthcare teams.
• Commitment to ongoing professional development and staying current with best practices in wound management.
• Demonstrate proficiency in technology usage, including mobile devices, applications, and camera functionalities, use of tele-medicine platform.
• Assess and meticulously document patients' wounds, capturing characteristics, measurements, and progress
• Develop and implement individualized wound care plans based on patient needs
• Administer appropriate wound care treatments, encompassing dressings, debridement, and topical medications
