Summary:
We are seeking a Registered Nurse – Case Manager for a 13-week travel assignment. This position is ideal for experienced RNs with a background in discharge planning, utilization review, and patient advocacy. The RN Case Manager will work closely with interdisciplinary teams to coordinate effective patient care and transitions.
Responsibilities:
Perform case management duties including assessment, planning, implementation, coordination, monitoring, and evaluation of options and services.
Collaborate with patients, families, and healthcare providers to ensure safe and timely discharge planning.
Monitor patient progress and resource utilization; advocate for patient needs.
Maintain compliance with all federal, state, and local regulations.
Document assessments and plans of care accurately using the facility’s charting system.
Participate in weekend coverage on an every-other-weekend basis.
Key Requirements:
Certifications: BLS (AHA)
Experience: Minimum 1 year of recent experience in case management as an RN
Skills Required: Knowledge of discharge planning, insurance/authorization, and patient advocacy
Additional Details:
Must be comfortable working independently and as part of a team
RTO must be submitted with application
First-time travelers are welcome
Local candidates accepted
Facility may call off up to once per two-week pay period
Qualifications
Education: ADN or BSN from an accredited nursing program
Experience: At least 1 year of recent case management experience in an acute care setting preferred
License: Active California RN License (Required)
Certifications: BLS required (American Heart Association)
Skills:
Strong critical thinking and problem-solving abilities
Familiarity with clinical pathways and patient care plans
Effective communication and interpersonal skills
