REQUIREMENTS:

* 2+ years of experience
* Current RN license in the state of practice
* Basic Life Support (BLS) certification

DUTIES & RESPONSIBILITIES:

* Providing compassionate care, strength and courage to patients and families.
* Helping patients identify their own personal goals of care and helping caregivers have confidence in caring for their family member.
* Acting as a generalist on all diseases and a specialist in symptom management, requiring excellence in assessment and critical thinking.
* Managing an assigned caseload of patients and leading the interdisciplinary team caring for those patients.
* Providing care wherever patients call home – private homes, nursing homes, group homes, or assisted living facilities.
* Maintaining a weekday schedule of Mon-Fri 8am-5pm with occasional back-up for weekends and on-call.
