Join Our Hematology/Oncology Team in Albuquerque Today!

We are looking for a dedicated RN Patient Navigator to manage and coordinate oncology care services. In this role, you will provide person-centered care coordination for patients in a clinic setting, working alongside clinical leaders to implement protocols and ensure high-quality service across the continuum of care. You will be responsible for navigating patient referrals, tracking outcomes, and maintaining communication between providers and community resources such as home health and palliative care.

• Current New Mexico Nursing License.


• Minimum of 2 years of experience in a healthcare setting.


• Experience with Chemotherapy, Oncology, or Care Coordination preferred.


• BSN in Nursing preferred.


• Proficiency in electronic medical records and strong critical thinking skills.
