To qualify for home health services, you must have a physician referral, and our case management coordinators are ready to assist in coordinating with your physician and health insurance for the approval process. Our services are tailored to fit every single patient's specific needs. Our goal is to provide you with a full range of services that help maintain your independence and dignity and support your loved ones. We use skilled nursing real-time advanced analysis and performance data to improve our operations and provide better care for our patients. All of our staff have completed background checks, meet all licensed home health qualifications, and are highly trained and supervised. Even when our nurses provide one-on-one home care to our patients, we utilize the latest electronic visit and rendered services verification system, as well as live patient monitoring technologies to ensure the safety and well-being of our patients. Our nurses and specialty therapists have immediate secured access to patients' medical records and are able to communicate and consult with our medical director or patients' physicians right from their home. We work with Medicare, Medicaid, and all HMO/PPO insurances, and our service coordinators can assist in consulting with your insurance provider regarding offered coverages and benefits.
