Embark on a Healing Journey in Scenic Fort Defiance!

Performs case management and coordinates the discharge planning care process of age-appropriate in-patients with chronic diseases and special health care needs within Multi-Service Unit, Intensive Care Unit, OB Units and Adolescent Care Unit by maintaining a tracking system for referred patients for admission with other healthcare facilities by performing chart reviews for requested services for return to facility or discharge home.

Two (2) Years or more experience in discharge planning and Med-Surg experience. Must have RN experience in a clinical, medical/surgical, acute and/or critical care setting.
