You will manage a caseload that allows you to spend meaningful time with each patient, building long-term therapeutic relationships and seeing the impact of your care over time. Patient acuity and visit frequency vary based on clinical needs, with a mix of post-acute, chronic disease, and rehabilitative cases common in the home health setting. While there are no traditional facility bed counts in home health, you will work within an organized structure that supports safe staffing and manageable visit volumes. Shifts generally follow daytime hours with some flexibility based on patient scheduling and agency needs, and may include occasional weekend or on-call rotation to provide continuity of care. The role requires comfort working independently in the field, sound clinical judgment, and a strong commitment to patient-centered care.
