Most assignments are based in Nassau County, with additional opportunities in Queens, Manhattan, and Brooklyn, giving you the chance to work with an interesting private-pay client population. In this role, you will perform in-home patient assessments, develop and update personalized care plans, and provide clinical supervision and guidance to our Certified Home Health Aides to ensure the highest quality of care. You will also collaborate closely with our case managers, coordinators, and families to maintain seamless communication and continuity of care. Active New York State RN license (required)
Minimum 1 year of home care nursing experience (required)
Strong communication and interpersonal skills
Ability to work both independently and as part of a team
Excellent organizational and clinical assessment skills
Reliable transportation to travel to patient homes in Nassau County, Queens, Manhattan, and Brooklyn
