Job Summary

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team on a PRN basis. The LPN will provide direct nursing care under the supervision of a Registered Nurse (RN) or physician, ensuring high-quality patient care in a rehabilitation, skilled nursing, or long-term care setting.

Key Responsibilities
Administer prescribed medications and treatments safely and accurately.
Monitor patient conditions and report any significant changes to the RN or physician.
Perform patient assessments, including vital signs and routine observations.
Assist with admissions, transfers, and discharges.
Provide wound care, catheter care, dressing changes, and other nursing procedures within the LPN scope of practice.
Assist patients with activities of daily living (ADLs), including bathing, dressing, feeding, and mobility.
Maintain accurate and timely documentation in the electronic medical record (EMR).
Educate patients and families on medications, treatments, and discharge instructions.
Collaborate with physicians, RNs, therapists, and the interdisciplinary healthcare team.
Follow infection control guidelines, safety standards, and facility policies.
Qualifications
Active Georgia Licensed Practical Nurse (LPN) License.
Current BLS (Basic Life Support) Certification.
Minimum 1 year of recent LPN experience preferred.
Experience in rehabilitation, skilled nursing, long-term care, or acute care preferred.
Strong communication, clinical assessment, and organizational skills.
Experience with Epic or other EMR systems is preferred.
