Position Summary
VitalBridge Health is seeking a reliable, compassionate Home Health Nursing Assistant (CNA or HHA) to provide direct, in-home patient care under the supervision of a Registered Nurse. This role supports patients with activities of daily living (ADLs), monitors basic health status, and ensures a safe and comfortable home environment.
Key Responsibilities
Assist patients with activities of daily living (bathing, dressing, grooming, toileting, feeding)
Provide mobility assistance and safe transfers
Monitor and report changes in patient condition to supervising nurse
Measure and document vital signs as directed
Assist with light housekeeping related to patient care
Support medication reminders (as permitted by state regulations)
Maintain accurate documentation per agency and regulatory standards
Promote patient dignity, safety, and comfort at all times

Qualifications
Required:
Active Florida CNA license or Certified Home Health Aide (minimum 75-hour training)
Current CPR/BLS certification
Valid Florida driver’s license and reliable transportation
Ability to pass Level II background screening
Excellent communication skills
Ability to work independently in a home setting
Preferred:
Minimum 1 year experience in home health or long-term care
Experience with geriatric or chronic disease patients
Bilingual (English/Spanish)
Work Environment
Patient homes throughout Orange County
