Position Overview
The Advanced Practice Provider (APP) will serve as the clinical lead for medical services within a residential substance use disorder treatment program. This role integrates direct patient care, detoxification oversight, medication-assisted treatment (MAT), interdisciplinary collaboration, and medical program leadership responsibilities.
The APP will provide comprehensive addiction medicine services while overseeing medical protocols, ensuring regulatory compliance, and supporting safe, effective, patient-centered care across all levels of treatment.
Core Responsibilities
Clinical Assessment & Diagnosis
Conduct comprehensive medical and substance use histories
Perform physical examinations at admission and as medically necessary
Assess intoxication and withdrawal severity
Conduct differential diagnoses for SUDs and co-occurring conditions
Order and interpret laboratory tests, toxicology screens, imaging, ECG rhythm strips, and other testing results.
Communicate diagnoses and treatment plans clearly to patients and families (with consent), and the multidisciplinary treatment team
Detoxification & Medication Management
Direct pharmacologic management of intoxication, withdrawal, and co-occurring biomedical and mental health conditions.
Initiate and manage Medication-Assisted Treatment (MAT), including but not limited to:
Buprenorphine products
Benzodiazepines (when clinically appropriate)
Naltrexone
Acamprosate
Psychotropic medications
Oversee all detoxification protocols, including vital sign monitoring, standardized assessments (COWS, CIWA), laboratory testing, and ongoing clinical evaluation.
Evaluate clients prior to initiation of MAT
Monitor medication adherence and respond to refusals
Ensure all medication management, documentation, and clinical services meet regulatory, licensing, and EHR standards which includes overseeing safe storage, documentation, administration, and return of medications
Incidental Medical Services (IMS) Oversight
Review and evaluate health questionnaires within required timeframes
Determine appropriateness of onsite medical services or need for referral
Oversee monitoring of client health status
Ensure required documentation is complete, signed, dated, and maintained
Ensure appropriate administration of injectable or implantable MAT when applicable
Treatment Planning & Care Coordination
Ensure medication plans include appropriate contraindications and precautions
Coordinate with The Lakes multidisciplinary treatment team, psychiatric providers, specialists, and external medical systems
Determine when higher levels of care or specialty referrals are required
Provide or arrange daily medical coverage
Be available onsite or on-call to respond to urgent medical or psychiatric situations
Program & Medical Leadership
Collaborate with program leadership in development and approval of:
Admission criteria
Detox protocols
Treatment protocols
Oversee adequacy of medical coverage
Support credentialing standards for prescribers and medical staff (as applicable)
Ensure compliance with regulatory and licensing requirements
Support staff education and supervision related to medical and detox services
Addiction Medicine Expertise
Demonstrate advanced knowledge of:
The ASAM Criteria
Epidemiology, neurobiology, and genetics of addiction
Co-occurring psychiatric and medical disorders
Addiction pharmacotherapies (OUD, AUD, TUD, StUD, CUD)
Harm reduction principles and overdose prevention
Trauma-informed and culturally responsive care
Social determinants of health
Treatment of complex co-morbidities requiring 24-hour monitoring
Patient-Centered Care & Professional Conduct
Deliver trauma-informed, culturally humble, stigma-free care
Communicate respectfully with patients, families, and staff
Apply de-escalation and behavioral management techniques
Respond to psychiatric decompensation and medical emergencies
Maintain ethical standards and professional boundaries
Comply with all state and federal privacy laws
Emergency Response
Respond to urgent biomedical and psychiatric situations
Manage substance-related emergencies
Maintain CPR certification
Preferred Experience
Experience in residential or detox treatment settings
Experience leading or coordinating interdisciplinary teams
Prior oversight of MAT programming
Experience with regulatory compliance in SUD treatment settings

Minimum Qualifications
Licensed Nurse Practitioner or Physician Assistant in good standing
Active prescriptive authority in the State of practice
DEA registration (as applicable)
Experience in addiction medicine, substance use disorders (SUD), and detoxification preferred
Working knowledge of The ASAM Criteria
Ability to act within scope of licensure under applicable state and federal regulations
Physical Requirements:
Work may be sedentary and mobile in nature. Must be able to sit, stand, walk hallways, climb stairs, and lift up to approximately 50 lbs. Reasonable accommodations may be provided when appropriate.
