Affirmed Home Care is seeking Per Diem Licensed Practical Nurses (LPNs) with pediatric tracheostomy and ventilator experience to join our compassionate, high-performing team throughout New Have County!

About the Role
As an Affirmed LPN, you’ll provide one-on-one, skilled nursing care to pediatric clients with complex medical needs in the comfort of their homes. This role is ideal for experienced nurses who are confident managing trach and vent care and are looking for meaningful, patient-focused work. We offer days/overnights and per-visit opportunities, allowing you to truly make an impact.

What You’ll Need
• Minimum 1 year of recent acute care LPN experience in Pediatrics
• Pediatric tracheostomy and ventilator care experience required
• Active Connecticut LPN license
• Current PALS Certification
• Eligibility to work in the U.S.
• Valid driver’s license (PREFERRED)
• Physical exam within the past 12 months
• PPD or QuantiFERON within 1 year, or chest X-ray within 5 years
