Position Summary :
The Director of Nursing (DON) provides clinical leadership and oversight for all patient care services. The DON is responsible for maintaining high standards of quality, regulatory compliance, patient outcomes, staff development, and operational excellence while supporting a collaborative and positive culture.
This leader will serve as a key member of the executive leadership team and work closely with clinical, operational, QA, intake, and support staff to ensure exceptional care delivery.
Essential Responsibilities :
Clinical Leadership
Lead, mentor, and support a multidisciplinary clinical team including RNs, LPNs, therapists, social workers, and home health aides.
Foster a culture of accountability, teamwork, and professional growth.
Provide guidance and support for complex clinical situations.
Promote evidence-based practices and strong clinical decision-making.
Quality & Compliance
Ensure compliance with Medicare Conditions of Participation, CMS regulations, accreditation standards, and agency policies.
Maintain survey readiness and support continuous performance improvement initiatives.
Monitor quality measures, patient outcomes, and key performance indicators.
Collaborate with QA and coding teams to improve documentation quality and regulatory compliance.
Operations
Oversee clinical operations to ensure efficient and effective patient care delivery.
Collaborate with intake, scheduling, and leadership teams to optimize patient services.
Support census growth while maintaining quality standards.
Assist in strategic planning and organizational development initiatives.
Staff Development
Recruit, develop, coach, and retain high-performing clinical staff.
Conduct performance evaluations and support professional development opportunities.
Promote employee engagement and a positive work environment.
Patient Care Excellence
Ensure patient-centered care remains the foundation of all clinical operations.
Address patient and family concerns promptly and professionally.
Support initiatives that enhance patient satisfaction and outcomes.
Requirements/Qualifications :
Active Registered Nurse (RN) license in the State of Michigan.
Minimum of 3 years of leadership or management experience in home health, hospice, or post-acute care.
Strong knowledge of Medicare regulations, Conditions of Participation, OASIS, and home health quality measures.
Demonstrated leadership, communication, and team-building skills.
Strong organizational and problem-solving abilities.
Preferred
Previous Director of Nursing or Clinical Manager experience in Medicare-certified home health.
Experience leading survey preparation and regulatory compliance initiatives.
Bachelor's degree in Nursing (BSN) or higher.
