Key Duties and Responsibilities
Assessment & Care Planning: Perform initial and ongoing, comprehensive head-to-toe physical assessments and develop, implement, and revise patient-specific care plans.
Skilled Nursing Care: Administer medications, manage pain, provide wound care (including complex, non-healing wounds), manage catheters, and perform IV therapy or tube feedings.
Monitoring & Reporting: Monitor vital signs, evaluate medication efficacy, and document changes in physical or cognitive conditions, reporting updates to physicians and families.
Patient/Family Education: Teach patients and caregivers about disease management, safety, infection prevention, and medication, ensuring a safe home environment.
Coordination & Compliance: Coordinate care with interdisciplinary teams (physical therapists, physicians, social workers), and maintain accurate, compliant patient medical records.
Core Responsibilities
Patient Advocate: Acting as the liaison between the patient, family, and medical team.
Safety Promotion: Assessing the home environment for safety hazards and recommending improvements.
Providing emotional support and counseling to patients and their families.

Registered Nurse License State of Florida
BSN preferred
2-3 years hospital experience
at least 1 year in home health
