Sanctuary Home Health Care LLC is seeking a compassionate, reliable, and skilled Licensed Practical Nurse (LPN) to join our growing home health team. This role is ideal for an LPN who enjoys working independently, providing one-on-one patient care, and supporting patients in the comfort of their own homes.

We are looking for someone who is patient-focused, dependable, and confident in clinical skills, with availability for midday and afternoon visits, including weekends.

Responsibilities

Provide skilled nursing care in patient homes

Administer medications, including insulin injections

Perform wound care, dressing changes, and basic assessments

Monitor patient condition and report changes to supervising RN

Accurately document visits in a timely manner

Educate patients and families on care and medications

Follow individualized care plans and agency policies

Maintain HIPAA compliance and patient confidentiality

Qualifications

Active LPN license in good standing

Experience in home health preferred (but willing to train the right candidate)

Strong knowledge of:

Insulin administration

Medication administration

Wound care

Ability to work independently and manage time efficiently

Compassionate, caring, and professional bedside manner

Reliable transportation and ability to travel between visits

Weekend availability required
