Central Peninsula Hospital is seeking an experienced Director of Care Coordination to lead our care coordination team and help improve quality of care, patient satisfaction, and healthcare outcomes.
This is an exciting opportunity for a qualified RN or Master’s-level Social Worker with leadership and case management experience.
This position is eligible for $25,000 relocation assistance, a Critical Position Bonus equal to $10,000 (1 year commitment) or $25,000 (2 years), and participation in the Director-level Incentive Compensation Plan, which currently provides a maximum annual incentive opportunity of up to 10% of annual earnings.
POSITION SUMMARY: The Director of Care Coordination oversees the clinical operations of the care coordination team. Instills strong management practices to create a culture of teamwork, collaboration, and timely achievement of objectives and goals. The Director of Case Coordination develops and integrates strategies to enhancing the quality of care for patients, satisfaction, and healthcare outcomes. They not only ensure patients receive proper care, but in a cost-effective manner for the patient and their families.

REQUIREMENTS:
Bachelors Degree in Nursing or Masters of Social Work
Alaska Licensure as a Registered Nurse or LMSW
2 years of leadership experience
2 years of case management or utilization review experience
