LPN - Home Health (PRN)

Position Summary:
The Home Health LPN provides skilled nursing care to patients in their homes under the supervision of an RN Case Manager. This role requires strong clinical judgment, excellent communication skills, and the ability to work independently in a field‑based environment. Proficiency with WellSky/Kinnser EMR and prior home health experience are essential. Note: This is a PRN position.

Work Environment:
This is a field‑based role requiring travel to patient homes. LPNs must be comfortable working autonomously while maintaining strong communication with the clinical team.

Required Qualifications:
Active LPN License in the state of Florida

Minimum 1 year of home health experience (field experience strongly preferred)

Proficiency with WellSky/Kinnser EMR

Strong assessment and critical‑thinking skills

Reliable transportation, valid driver’s license, and proof of auto insurance

Ability to work independently in a home‑based setting

Prior work in a Medicare‑certified home health agency
