Physical Therapist

Job Title: Physical Therapist (PT) – Home Health
Location: Knoxville, TN 37922
Hours: 40 hours/week | 8 hours/day
Schedule: Monday–Friday, 8:00 AM – 5:00 PM (No weekends, on-call and overtime required)
Mileage Reimbursement: $0.41 per mile
Perform physical therapy evaluations and treatments per physician orders and referrals
Conduct comprehensive patient assessments using appropriate evaluation tools and procedures
Review patient diagnoses, medical history, precautions, and contraindications
Develop individualized, measurable treatment plans to maximize patient independence and functional outcomes
Reassess patient progress regularly and modify treatment plans as clinically indicated
Communicate patient needs and progress with physicians, interdisciplinary teams, patients, and family members

Home health physical therapy experience
Homecare Homebase (HCHB) experience
Vestibular experience preferred
License/Certification:
Physical Therapy License (Required)
