As an RN Case Manager, you will:
• Perform patient assessments and develop individualized care plans
• Provide skilled nursing visits in patients’ homes
• Educate patients and caregivers on disease management and medications
• Monitor patient progress and communicate with physicians and care team
• Supervise LVNs and home health aides as needed
• Complete timely documentation (OASIS experience preferred)
• Participate in on-call rotation and quality improvement initiatives

Skills You May Use (Training Available!):
We welcome RNs with experience — or interest in learning — skills such as:
• Wound care (Wound Vacs, Unna Boots)
• PICC line & Medi-Port care
• G-Tube, NG Tube, and trach care
• Post-surgical wound management
• Inotropic therapy
• Dialysis patient medication management
We provide training to help you expand your clinical skills.

Requirements

• Active Texas RN license
• At least 1 year of nursing experience (home health preferred but not required)
• Strong communication and documentation skills
• Compassionate, patient-focused mindset
• Reliable transportation for home visits
