StartDate: 8/31/2026 Pay Rate: $1534.00 - $1613.00

Case Manager needed for 350 bed Level 3 Trauma center. Discharge Planning, SNF referrals, LTAC, HH, NH, HD placement, DME, Hospice, Family meetings, large unfunded and under- resourced population Suburb 15 miles southeast of Houston, close to gorgeous Gulf beaches

Required Qualifications
Registered Nurse, Case Manager
Experience: 1 year
Licenses: RN(Compact Accepted)
SSN Required
DOB Required
Work History: Should not be more than 90 days of gap in last 1 year

Preferred Qualifications
Meditech, MIDAS, NAT
