Plan, organize, and direct hospice care with emphasis on community health education/experience. Build from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
• Assume responsibility for notifying the Clinical Supervisor or following up as needed in a timely manner during business hours
• Fulfill the obligation of requested and/or accepted case assignments
• Report all information to team at end of shift and sync back electronic documents in a timely fashion
• Complete an initial, comprehensive, and ongoing assessment of patient and family to determine hospice needs.
• Provide a complete physical assessment and history of current and previous illness(es)
• Provide professional nursing care by utilizing all elements of nursing process
• Initiate the plan of care and make necessary revisions as patient status and needs change
• Use health assessment data to develop a care plan that establishes goals, based on nursing diagnosis, and incorporates palliative nursing actions
• Include the patient and the family in the planning process
• Administer medications and treatments as prescribed by the physician in the physician’s plan of care
• Counsel the patient and family in meeting nursing and related needs
• Provide health care instructions to the patient as appropriate per assessment and plan
• Assist the patient with the activities of daily living and facilitate the patient’s efforts toward optimal comfort care
• Act as Case Manager when assigned by Clinical Supervisor and assume responsibility for coordinating patient care for assigned caseload
• Complete, maintain, and submit accurate and relevant clinical notes regarding patient condition and care given
• Record pain/symptom management changes/outcomes as appropriate
• Follow up as needed and appropriately with management staff pertaining to any concerns regarding patient care
• Communicate with the physician regarding the patient’s needs and report changes in the patient’s condition; obtain/receive physicians’ orders as required
• Communicate with community health-related persons to coordinate the care plan
• Stay up to date with current memos and updates to system, read emails, and communicate with team
• Complete documentation accurately within 24 hours
• Attendance to all staff meetings required unless absence is approved by the Supervisor
• Assume responsibility for personal growth and development and maintain and upgrade professional knowledge and practice skills through attendance and participation in continuing education and in-service classes
• Fulfill the obligation of requested and/or accepted case assignments
• Actively participate in quality assessment performance improvement teams and activities
• Prepare and maintain clinical documentation according to Agency policies and acceptable nursing standards

• Perform other duties as assigned by supervisor
Hospice Registered Nurse, After Hours Qualifications:
• Associate degree from an accredited School of Nursing
• A minimum of two (2) years of experience working as a Registered Nurse, including Hospice experience
• Hold, in good standing, a license as a Registered Nurse or Licensed Practical Nurse, issued by the state in which you work
• Able to meet the physical demands of providing nursing care, ambulating patients, transferring patients, etc.
• Able to transfer patients as needed according to proper techniques of body mechanics to promote patient and staff safety
• Able to use good and sound clinical judgment
• Able to document accurately and appropriately
• CPR certified
• Empathetic, non-judgmental, tactful, responsible, and organized
• Understand the team approach and be a supportive team member
We are looking for compassionate nurses with:
RN license in the state you work
Associate degree or higher from an accredited School of Nursing
Two years of RN experience, hospice experience preferred
Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
A commitment to consistently meet critical deadlines for charting
The skills needed to self-manage your time and schedule
Demonstrated experience with tablets, mobile phones and EMR software
