Role Overview
The RN Case Manager coordinates care for health plan members by assessing needs, creating care plans, connecting members to resources, and supporting them across the care continuum. Communication includes phone, email, text, and digital platforms.

Key Responsibilities

· Coordinate care with a multidisciplinary team (Social Workers, Dietitians, Pharmacists, Medical Directors).

· Assess members’ medical, psychosocial, and support needs.

· Develop and manage individualized care plans and address barriers to care.

· Connect members to community, behavioral health, and disease-specific resources.

· Educate members on medications, self-management, and health literacy.

· Support transitions of care, including discharge planning.

· Document all interactions and follow quality/protocol standards.

· Proficiency with Microsoft Office and digital tools.

Requirements

· Nursing Diploma or Associate Degree required; BSN preferred.

· 3+ years clinical RN experience (acute, post-acute, or community).

· Case management in managed care preferred.

· Experience working with patients by phone/digital platforms preferred.

· Active Compact RN License required.

· CCM certification required (or within 18 months); CCP preferred.
