Job Title: RN Hospice Case Manager
Location: Barre, VT 05641
Duration: 20 weeks (with possibilities of extension)
Shift: Monday - Friday
Guaranteed Hours: 36 hours
Overtime Starts after 40 hours
Pay Details:
For Travelers/Stipend Candidates:
Blended Hourly Pay Rate (W2): $73.00
Blended Weekly Gross Pay Rate: $2,628.00
Requirements:
Experience with hospice case management required
Experience providing hospice care in a home setting required.
Prior experience with rural travel preferred.
Experience with McKesson/Netsmart EMR preferred
BLS (AHA)
