Join our LoveWell team as a Full-Time RN Case Manager serving patients and families throughout the Lewisville/Denton area. In this role, you'll provide expert clinical care, compassionate support, and guidance to patients and their loved ones during some of life's most meaningful moments.

As an RN Case Manager, you will coordinate and oversee patient care, collaborate with the interdisciplinary team, and serve as a trusted resource for patients and families. The ideal candidate combines strong clinical skills with emotional intelligence and a genuine passion for hospice care.

-Active Texas RN license required
-Reliable transportation, valid drivers license and current auto insurance required
-2 years of experience with a Hospice agency preferred
-FireNote experience preferred
-Ability to read, write, and speak the English language
