**Must have weekend availability** Their duties may include: -Case managing, DC planning and clinical review for KP members who are admitted for Skilled care at a Nursing Facility -Call backs for members recently DCd from the hospital, ED and SNF -Telephonic assessments of our Dual Special Needs population -Case managing, DC planning and clinical review for KP members who are admitted for Skilled care at a Nursing Facility -Call backs for members recently DCd from the hospital, ED and SNF -Telephonic assessments of our Dual Special Needs population
