Job Summary:
The RN Home Health is responsible for providing an initial, resumption, and recertification evaluation of the Home Health patient in their home setting, along with other routine home health visits. This includes age appropriate interventions that meet physical, emotional, developmental, psychosocial, and educational needs. The RN Home Health is responsible for the coordination of initial disciplines to begin care, including the planning of care throughout the patient episode, collaborating with the physician, patient, family member and team manager to develop the expected patient visits, scheduling the visits to the appropriate team member, incorporating therapeutic, preventative and rehabilitative nursing actions. The RN Home Health communicates with all team members including RNs, LPNs, Therapists, Social Workers, Home Health Aides, the Physician, patient, family, and other internal or external individuals to ensure that the patient's plan of care is appropriately developed.

Epic Remote or Home Care Home Base Systems; Wound Vac’s; Wound Care; IV Management; Assess and Teach disease process ie CHF; COPD etc; Catheter insertion and care; PICC line dressing changes;
Required Minimum Education:
Associates Nursing or Diploma (Nurse) Nursing or Bachelors Nursing-Preferred
Required Minimum License(s) and Certification(s):
All certifications are required upon hire unless otherwise stated.
RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact
BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support - Instructor
GADL - Georgia Driver's License or VSDL - Valid State Driver's License
PVI - Private Vehicle insurance
Additional License(s) and Certification(s):
Required Minimum Experience:
Minimum 1 year Medical/Surgical Nursing Required or Home Health, and ICU experience Preferred or Must possess extremely strong critical care nursing skills Required
Required Minimum Skills:
Computer/data entry experience. Ability to communicate with various members of the healthcare team. Ability to use EXCEL, Word and have basic computer operational knowledge Epic and 3M experience preferred.
