REQUIREMENTS:

* Must hold active LPN/LVN license in practicing state.
* LTC/SNF/Assisted Living experience preferred.
* Strong clinical, communication, and care skills.
* Able to follow care plans and collaborate with staff.

RESPONSIBILITIES:

* Clinical Care:

* Assist with ADLs and monitor resident health status.
* Administer medications and treatments.
* Document resident care and report all changes.

* Collaboration & Safety:

* Follow physician orders and established care plans.
* Promote a safe, clean, and supportive environment.
* Provide emotional support and companionship.
