Home Health Clinical Director

Description

Lamoille Home Health & Hospice is seeking a mission-driven, collaborative, and visionary Clinical Director to lead and oversee clinical operations across our home health programs. The Clinical Director will ensure the delivery of high-quality, patient- and family-centered care in the home and community setting while maintaining full compliance with CMS Conditions of Participation and applicable Home Health regulations, state regulations, and organizational standards.
The ideal candidate demonstrates strong clinical leadership, regulatory knowledge, and a deep commitment to compassionate, community-based care. This role partners closely with the Hospice Director and Executive Director and interdisciplinary leadership to support staff, drive quality outcomes, and advance our mission of serving our community with dignity, respect, and excellence.

Requirements

• Licensed Registered Nurse (RN) preferred; other appropriate clinical licensure considered.
• Minimum of 2 years of progressive clinical leadership experience in home health, hospice, or community-based healthcare preferred.
• Strong knowledge of CMS regulations, including 42 CFR §418 Hospice Conditions of Participation.
• Demonstrated experience with quality improvement, regulatory compliance, and survey readiness.
• Proven ability to lead interdisciplinary teams in a patient-centered care environment.
• Excellent communication, leadership, and organizational skills.
