Job Summary:
We are seeking a compassionate and skilled Registered Nurse (RN) to provide high-quality nursing care to patients in their homes. The RN will assess patient needs, develop and implement individualized care plans, and collaborate with other healthcare professionals to ensure continuity of care. This role is ideal for a nurse who thrives in an autonomous, patient-centered environment.

Key Responsibilities:
Perform initial and ongoing assessments of patients’ clinical conditions and home environments.

Develop individualized care plans based on assessments, physician orders, and patient goals.

Administer medications, wound care, IV therapy, and other skilled nursing interventions.

Educate patients and families on disease processes, self-care techniques, and preventive health strategies.

Coordinate and communicate effectively with physicians, therapists, and other healthcare team members.

Monitor patient progress and update care plans as needed.

Document all care provided and maintain accurate, timely records in compliance with HIPAA and home health regulations.

Respond promptly to changes in patient condition and take appropriate clinical action.

Participate in on-call rotation and emergency visits as needed.
