Job Title: Licensed Practical Nurse (LPN)

Job Summary:
The Licensed Practical Nurse (LPN) in a Long Term Care (LTC) facility is responsible for providing direct nursing care to residents under the supervision of a Registered Nurse (RN) or physician. The LPN ensures residents receive quality care, medication administration, and support with daily healthcare needs while maintaining compliance with healthcare regulations and facility policies.

Key Responsibilities:

Administer medications and treatments as prescribed
Monitor and document residents’ health conditions and vital signs
Assist in developing and implementing patient care plans
Provide wound care, catheter care, and other nursing procedures
Respond to residents’ medical needs and emergencies
Supervise and support Certified Nursing Assistants (CNAs)
Maintain accurate patient records and documentation
Communicate effectively with residents, families, and healthcare teams
Ensure compliance with LTC policies, infection control, and safety standards
Support residents with activities of daily living when required

Requirements:

Active LPN/LVN license
BLS/CPR certification preferred
Experience in Long Term Care or Skilled Nursing preferred
Strong communication and patient care skills
Ability to work flexible shifts, including weekends and holidays

Preferred Skills:

Knowledge of geriatric care
EMR/EHR documentation experience
Team-oriented and compassionate approach
Strong time management and organizational skills
